With mullvad you can use their own dns or any other dns provider.
I feel like theirs is pretty trustworthy given the rest of their service. They also go above and block a bunch of crap like malware and gambling 👍
Yes, I’ve been using their services for years and I am very happy about them, they are cheap, I can connect 5 devices, I can choose to block ads, adult material or gambling, great for for my children, also their app work well on Linux and Android and they look trustworthy.
Why not? Some VPN providers operate their own network of DNS servers to avoid leaks via DNS, e.g. mullvad.

I would imagine it depends on the provider. I know you can change to a custom DNS in IVPN, AirVPN and Mullvad.
It’s going to be dependant on the VPN, but some definitely allow for you to choose a DNS server. Depending on your OS, you can just setup all traffic to be routed through a specified DNS server too. This is one aspect of Fedora I really dig.
